Police Checks
Police Checks for volunteers are valid for 3 years.
If you require an updated police check, please request a Vulnerable Sector Check. These checks can be done free of charge at the OPP website here: OPP Record Check
You will require a letter from the organization to support this request. You can download the EMHA letter here: 
EMHA_2024_Police_Check_letter_final **please note you will have to fill in your name and position you are applying for on the letter.
All completed police checks need to be submitted to the EMHA Coaches Convenor
If Police Check is current and will not expire this hockey season, an annual Declaration of Criminal Record Form can be completed. Declaration form can be found here: EMHA Declaration of Criminal Record Form
